Job summary

The Grand Rehabilitation and Nursing at Mohawk Valley is seeking LPNs to join our team. Full-time or part-time positions with day, evening, and night shifts are available, including Baylor weekend options. Pay range is up to $37.21 per hour, with 8- or 12-hour shifts to suit your schedule.

Responsibilities include patient care, accurate documentation, supporting the CNA team, and medication administration. NY LPN license and valid CPR are required; prior experience in a nursing home or hospital

Qualifications

• Current NY LPN license and CPR certification required.
• Experience in a Nursing Home or Hospital is a plus.
• 2+ years of LPN experience preferred (if stated).

Responsibilities

• Provide patient care and support residents and families.
• Complete documentation using the electronic medical record system.
• Support CNA team to ensure quality personal care and accurate CNA documentation.
• Administer medications and report as required by protocols.

The Grand Rehabilitation and Nursing at Mohawk Valley is seeking LPNs to join our team!

Position type: Full-time or Part-time

Pay range: up to $37.21

Schedule: Days, evenings, nights available, flexible scheduling available. We also have Baylor (work 4 shifts on the weekend and get paid for 5 shifts) availability. 8 or 12 hour shifts available.

LPN Responsibilities:

• Patient Care & Service: Promote and restore patients' health by completing the nursing process; perform various treatment procedures; provide physical, educational, and emotional support to patients, friends, and families; supervise assigned team members

• Documentation: Ensure concise, pertinent, and complete resident care documentation using computerized medical record process

• Supports CNA team: Direct the CNA in the timely completion of quality individualized personal care and ensure CNA documentation is accurate and complete

• Medication Administration & Reporting: Administer medication and give/receive relevant reports, both written and verbal from/to appropriate staff as per departmental protocols

LPN Requirements Include:

• Current and valid NY LPN License

• Experience in a Nursing Home or Hospital is a plus

• Valid CPR Certification
