Job summary
Spring Creek Rehab in Murray, Kentucky is seeking a Licensed Practical Nurse (LPN) to join our team and provide bedside care, medication administration, and patient education. The role includes collaboration with physicians to plan care and ensure high-quality outcomes.
We offer new pay rates, a Full Time Commitment Program with $1,000 quarterly payouts for 3 years, a Weekend Baylor incentive, and benefits such as 401(k), health, dental, vision, life insurance, and PTO.
Qualifications
- Diploma in Nursing at a minimum.
- Licensed as an LPN.
- Ability to safely and successfully perform job-related functions.
Responsibilities
- Provide direct bedside care to patients, including vital sign monitoring, wound care, and medication administration.
- Educate patients and family on aging or specific conditions and promote active participation in recovery and long-term wellness.
- Inform family of changes in patient condition and discuss medication or treatment alterations with them.
- Collaborate with facility physician to develop and implement patient care plans and notify physician of changes.
- Maintain accurate, up-to-date records and communicate effectively with other healthcare professionals.

- Provide direct bedside care to patients, including but not limited to vital sign monitoring, wound care, and medication administration
- Educate patients and family members on what to expect when dealing with the challenges of aging or specific conditions, and encourage them to take an active role in their recovery and long- term wellness
- Inform family members of any changes in the patient's condition and discuss with them any alterations to the patient's medication or other treatments
- Collaborate with the facility physician to develop and implement patient care plans, and notify the physician of any changes in the patient's condition
- Maintain accurate and up-to-date records of patient care and communicate effectively with other healthcare professionals
