Job summary

Wexford House in Kingsport, TN is seeking a compassionate LPN to join our care team. You will be a key member of our residents’ care, directing CNAs, managing medications, and supporting care plans in a team-focused environment.

Responsibilities

• Direct the daily activities of CNAs and ensure nursing policies and procedures are followed.
• Participate in admissions, discharges and transfers of residents as required.
• Assess residents’ conditions, prepare and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report changes in a resident’s condition to the nurse supervisor and keep them informed of factors affecting work.
• Perform other duties assigned by clinical or administrative leadership.
• Maintain confidentiality of protected health information.

LPN Position Summary

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
