Four Oaks Healthcare Center in Jonesborough, Tennessee is seeking an LPN to join our care team on the night shift. This full-time role involves leading CNAs, managing admissions and transfers, and administering medications under physician orders.

You will contribute to care planning, monitor residents, and report changes to nursing leadership while maintaining patient confidentiality and delivering compassionate care.

Qualifications

• Current CPR/BLS Certification.
• Current unrestricted LPN license for Tennessee.

Responsibilities

• Direct the daily activities of CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess residents' conditions, prepare and administer medications per physician orders.
• Initiate, review, and update care plans.

LPN Position Summary

As a LPN at Four Oaks Healthcare, you'll be an essential part of our residents' care. Responsibilities include:

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.

• Participate in admissions, discharges and transfers of residents as required.

• Assess resident's conditions, prepare, and administer medications per physician orders.

• Initiate, review, and update care plans.

• Report any changes in a resident's condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.

• Perform other assigned duties given by clinical or administrative leadership.

• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

LPN Requirements

• Current unrestricted LPN License for the state of Tennessee

• Current CPR/BLS Certification
