Job summary

Creekside Center for Rehabilitation and Healing in Tennessee is seeking a licensed Occupational Therapist for a part-time role. You will assess referred patients, develop and administer rehabilitation plans, and set achievable goals with patient capacity in mind.

You will supervise Occupational Therapy Assistants, document conditions and progress, educate patients and families, and collaborate with all disciplines to deliver patient-centered care, with strict confidentiality and professional

Qualifications

• Must have graduated from an accredited OT program.
• Must hold a valid OT license and state registration.
• Experience supervising Occupational Therapy Assistants.
• Experience in long-term or geriatric care preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and determine goals.
• Establish and administer treatment plans with physician direction.
• Set realistic goals based on patient capacity and tolerance.
• Document patient condition and progress, including discharge notes.
• Educate patients and families in therapy approaches.
• Collaborate with other disciplines to provide patient-centered care.
• Oversee and supervise Occupational Therapy Assistants.
• Ensure confidentiality and HIPAA compliance while documenting.

Occupational Therapist Part Time

The Creekside Center for Rehabilitation and Healingislooking for a talented and hard-working Occupational Therapist to join our ever-growingteam.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.

• Must have a valid professional license and registration for state of practice.

• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.

• Experience with long-term care and/or geriatric care is preferred but not limited to.

• Well versed with documentation writing

• Evaluations, weekly progress notes, re-certification and discharge documentations.

• Collaborates with all disciplines to provide best patient centered treatment and care.

• Exceptional patient service skills and ability to demonstrate clinical excellence

• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.

• Perform all other duties assigned by Director of Rehabilitation

• Compliance with confidentiality, HIPPA and Federal Residents Rights.
