Job summary

Trilogy Health Services is seeking an LPN to provide direct clinical care and supervise nursing assistants at the Corydon facility. The role focuses on observing residents, reporting changes, and delivering compassionate, holistic care daily.

You will support medication and supply management, complete essential documentation, and collaborate with the nursing team to ensure resident well-being and safety. LPN licensure and CPR certification are required.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current CPR certification required.

Responsibilities

• Visit residents daily on assigned unit to assess health status and notify appropriate staff of changes.
• Identify problems and emergencies and initiate lifesaving measures in absence of physician.
• Provide holistic care within LPN scope.
• Monitor medications and supplies to ensure adequate inventory.
• Complete medical forms, reports, and charting as needed.
• Report exposures per facility policy.
