Job summary

Mosaic Life Care is seeking an LPN to provide quality patient care under supervision throughout our northwest Missouri system. You will administer medications, monitor responses, and support care plans in collaboration with RNs and providers.

The role emphasizes patient education, accurate documentation, and teamwork to deliver consistent, compassionate care across multiple facilities. Travel between locations is required as needed.

Qualifications

• Degree from an accredited school of practical nursing, or military equivalent.
• Licensed Practical Nurse (LPN) – State Licensure – required upon hire.
• IV Certification – required upon hire.
• Basic Life Support (BLS) – required within 90 days.
• Advanced Cardiac Life Support (ACLS) – required within 90 days.
• Willingness to travel between facilities as needed.

Responsibilities

• Completes daily reassessments for assigned patients and documents findings.
• Updates and implements the plan of care.
• Reinforces patient teaching and family education.
• Administers medications and monitors patients’ responses to medications including intravenous fluids and medications.
• Collaborates effectively with all members of the interdisciplinary team.
• Contributes to the professional development of peers, colleagues, and others (students, orientees).
• Other duties as assigned

• Must walk, move, kneel, crouch, stoop, reach, stand, pull, push, and lift, speak clearly

Essential Sensory Requirements

• Ability to visually assess patient condition; must be able to communicate with patient in order to assess condition and evaluate effects of care.
• Visual, hearing, feeling, smell.

Exposure to Hazards

• Body fluids, blood, punctures, scratches, and physical injury from combative patients, radiation exposure.
• Needle sticks, cuts, back injury, infection/disease exposure from patients, visitors, and co‑workers.
