Job summary

Catholic Charities Brooklyn and Queens seeks a Licensed Practical Nurse to join the ACT team. The LPN will support the RN, conduct health screenings, and assist with medication management in collaboration with the psychiatrist and RN, delivering intensive in-home and community-based services to clients with severe mental illness.

As part of a multidisciplinary team, you will perform nursing care, monitor vital signs, document progress notes, and provide crisis intervention as needed.

Qualifications

• LPN with NY license required.
• ASDN or equivalent required.
• Bachelor’s degree preferred.

Responsibilities

• Work with ACT team as part of an interdisciplinary group.
• Provide nursing care and in-home visits.
• Perform health assessments and referrals.
• Monitor medications and report to physicians.
• Maintain progress notes and required documentation.
• Participate in 24/7 on-call rotation.
• Crisis intervention as needed.
