Job summary

Hartford HealthCare Medical Group is seeking a Licensed Practical Nurse to provide direct patient care within a unit or service, collaborating with the care team to meet patient goals and document data as part of the care plan.

You will perform therapeutic measures under the direction of an RN or MD to support patient outcomes. Connecticut LPN license is required; medical/surgical office experience is preferred.

Qualifications

• Current Connecticut Licensed Practical Nurse license required.
• Specialization and/or specific work experience may be required.

Responsibilities

• Collaborates with members of health care team to assist patients in achieving established outcomes of care.
• Participates in direct patient care in order to meet individual and family health goals by: obtaining, documenting and reporting patient data related to established plan of care
• Performs therapeutic measures, under the direction of an RN or MD, to assist in achieving established patient care outcomes.
