Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking a Licensed Practical Nurse (LPN) to join our Skilled Unit and Long Term Care Unit. You will provide patient care, administer medications (including IVs), and monitor residents to ensure positive outcomes.

You will educate residents and families, communicate with physicians and NPs, and supervise CNAs. A current LPN license is required, with a supportive team and opportunities for growth.

Qualifications

• Current LPN license required.
• Experience with IV medications preferred.
• Ability to educate residents and families.

Responsibilities

• Medication administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs.
