Job summary

Wellpath is seeking a Licensed Practical Nurse (LPN) to provide compassionate, patient-centered care in a supervised setting within a healthcare facility in Kentucky. The role emphasizes assessment, medication administration, and adherence to safety and documentation standards, contributing to quality patient outcomes.

Applicants must graduate from an accredited nursing program, hold active LPN/LVN licensure in the state of employment, and preferably have at least one year of clinic experience.

Qualifications

• Graduate from an accredited school of nursing.
• Prefer a minimum of one (1) year clinic experience.
• Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse in the state of employment.

Responsibilities

• Under RN supervision, assess patients' physical, psychological, and social dimensions to support individualized treatment planning.
• Administer medications, including IV therapy if certified, and perform diagnostic tests per protocols.
• Participate in controlled substance counts at shift start and respond to health emergencies.
• Assist in procedures, implement nutrition and therapeutic diets, and document nursing encounters using SOAP charting.
• Attend mandatory staff meetings and trainings to ensure high-quality patient care.
