Job summary

Covenant Health HomeCare and Hospice is seeking a Licensed Practical Nurse to provide nursing care to homebound patients under the supervision of an RN.

This full-time role includes performing comprehensive cardiovascular, pulmonary, neurological, GI and skin assessments; medication administration; IV therapy with certification; colostomy and tracheostomy care; and coordination with the clinical team. Local travel is required, with day shifts and a comprehensive benefits package.

Qualifications

• One year of professional experience in a hospital/clinical setting or prior home health experience.

Responsibilities

• Perform comprehensive cardiovascular, pulmonary, neurological, GI and skin assessments.
• Administer medications per HomeCare policy; check allergies and MD orders.
• Perform IV procedures with certification; venipuncture and infusion management.
• Provide colostomy and tracheostomy care and suctioning.
• Collaborate with RN, Clinical Supervisor, and other disciplines to update the plan of care.
• Travel locally to patient homes as required.
• Follow infection control and safety policies and complete annual education.
