As the Home and Community Based Services Licensed Practical Nurse, you will be providing direct nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes, recording clinical and progress notes, and coordinating other patient care activities or services to provide the highest quality of care in the geographic service area assigned. You will work under the direct supervision of the Registered Nurse and/or designated supervisor.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. The hourly pay for this role will range from $20.38 to $36.44 per hour based on full-time employment.
