Job summary

Angels Care Home Health is seeking a dedicated Licensed Practical Nurse to administer skilled nursing care under the supervision of a registered nurse in clients' homes. You will coordinate with the interdisciplinary team to ensure high-quality care that meets patient needs.

The ideal candidate holds an LPN license, valid driver's license, CPR certification, and at least two years of LPN experience, preferably in home health.

Qualifications

Graduate of an accredited practical/vocational nursing program.
Current LPN license in the state.
Valid driver's license in good standing.
Minimum two years of LPN experience; home health preferred.
Proof of current CPR and Hepatitis profile.

Responsibilities

Administer skilled nursing care under RN supervision in patients' homes.
Coordinate care with interdisciplinary team, patients, families, and referring agencies.
