Qualifications

• Holds a current Licensed Practical Nurse license in Virginia.
• Minimum of one year of nursing experience.
• Current CPR certification.
• Excellent communication, organizational, and human relations skills.
• Able to function with minimal supervision and manage changing priorities.
• Able to employ critical thinking skills.

Responsibilities

• Accepts assignments of clients from the R.N. Case Manager and provides nursing care per care plans.
• Performs physician-ordered treatments delegated by the R.N. Case Manager.
• Observes signs and symptoms and reports them to the R.N. Case Manager.
• Notes changes in physical or emotional status of clients and promotes independence in self-care.
• Utilizes electronic documentation and adheres to agency policies and safety guidelines.
