Job summary

RVNAhealth is seeking a Licensed Practical Nurse (LPN) to provide hands-on nursing care in patients' homes within Central Fairfield County, CT, including Ridgefield and surrounding towns. Under RN supervision, you will advocate for patients, follow care plans, document accurately, and participate in ongoing professional development.

The role requires a CT LPN license, at least 2 years of clinical home care experience, and reliable transportation to travel between assignments.

Qualifications

  • Graduate of an accredited nursing program.
  • Active LPN license in the State of Connecticut.
  • Minimum of 2 years clinical nursing experience (home care preferred).
  • Strong understanding of medical terminology and Medicare/insurance documentation requirements.
  • Calm under pressure with strong organizational and multitasking skills.
  • Excellent interpersonal communication and customer service skills.
  • Proficient with electronic health records and computer documentation.
  • Must have reliable transportation and the ability to travel in all weather conditions.

Responsibilities

  • Build strong, trust‑based relationships with patients, families, and team members.
  • Support agency quality improvement efforts and participate in ongoing professional development.

About the Role:

As an LPN, you'll provide hands‑on nursing care directly in patients' homes, supporting individuals through acute, chronic, or rehabilitative health challenges.
