Job summary

Aveanna Healthcare, LLC in Colorado is hiring Licensed Practical Nurses (LPN/LVN) to provide skilled nursing care in patients' homes. You will work under a Registered Nurse, follow care plans, document care, and maintain confidentiality.

We offer full-time, part-time or PRN positions with AM/PM shifts, competitive pay ($24-$28/hr), sign-on bonuses, benefits including health, dental, vision, life insurance, PTO, and 24/7 local support. Training and career growth available.

Qualifications

Must have and maintain an active, unencumbered LPN license.
Current CPR certification (hands-on component). Aveanna can assist after hire if needed.
TB skin test current within last 12 months.
Six months hands-on nursing experience preferred but not required.
Must have reliable transportation.
SC pediatrics experience (1 year).
CA nursing license experience (1 year).
LA nursing experience (1 year).
Continuing Education as required by state.
Six months recent home health experience preferred.

Responsibilities

Deliver and coordinate quality patient care in line with physician orders.
Observe and assess patient condition and report changes as appropriate.
Document activities, assessments, nursing actions, and care coordination in a timely manner.
Participate and update the nursing care plan.
Take appropriate nursing actions and achieve outcomes.
Recognize changes in patient needs and intervene to reduce risk.
Adhere to Aveanna policies, state/federal regulations, and Nurse Practice Act.
