Job summary

All Ways Caring HomeCare seeks a Licensed Practical Nurse to provide direct and indirect care to clients at home. You will document nursing actions, administer medications, and conduct assessments under physician orders and plans of care.

The role supports home-based care, collaborates with the Nurse Supervisor and Branch Manager, and participates in staff meetings while maintaining compliance with regulatory standards.

Qualifications

• Degree in Nursing from an accredited school.
• One or more years of nursing experience preferred.
• Current and unrestricted license in the state of operation.
• CPR professional certification.
• Excellent communication skills with all levels of personnel internal and external to the company.
• Capable of working independently with highly confidential information.

Responsibilities

• Provides skilled nursing care per Physicians Orders and Plan of Care.
• Documents nursing actions and informs physicians and family accordingly.
• Administers and monitors medication as prescribed by physician.
• Conducts nursing assessment visits as needed.
• Follows policies and procedures according to operation, state and regulatory standards.
• Completes supervisory visits to review plan of care and caregiver competency.
• Implements nursing support for individuals in a home setting.
• Works with Director or Nursing/Nurse Supervisor and/or Branch Manager to address health needs.
• Participates in staff meetings and other required meetings.
• Ensures compliance with physician orders and medical reports.
