Job summary

Masonicare Home Health & Hospice in Plainfield, CT is hiring an LPN for day shift, 40 hours per week to provide skilled nursing care in home care, hospital or extended care settings as directed by the Nurse Case Manager.

The LPN will coordinate care with the registered nurse, document care and outcomes, and educate patients and families while maintaining compassionate, high-quality care and complying with agency policies and state regulations.

Qualifications

• NLN-accredited nursing program graduate.
• Active Connecticut Practical Nurse license.
• At least 1 year of acute care experience; homecare preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Coordinate care with community resources and the nurse case manager.
• Provide skilled nursing care to adult, geriatric, pediatric and neonatal clients.
• Document care and outcomes in a timely, accurate manner.
• Educate patients, families and caregivers about care plans and treatments.
• Report client issues to the designated nurse promptly and professionally.
• Adhere to agency policies, safety procedures and regulatory requirements.
