Job summary

Beaumont Home Care in Detroit seeks a Licensed Practical Nurse (LPN) to deliver skilled nursing care in patients’ homes. You will follow physician orders, monitor patient conditions, administer medications, and document care.

The role requires an active LPN license and at least 1 year of acute care experience (home health preferred). Strong communication and compassionate patient care are essential, with on-call duties as part of the schedule.

Qualifications

• Licensed Practical Nurse with active license in the state of employment.
• Minimum 1 year of LPN experience in acute care; 2 years preferred.
• Home care experience desired.
• Effective communication with patients, families, physicians and co-workers.
• Compassionate, patient-centered and able to remain calm.
• Knowledge of nursing best practices and clinical judgment.
• Ability to follow directions and work within a team.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provides skilled nursing care in patients' homes as ordered by physician.
• Administers medications as prescribed and follows order sets.
• Observes and documents patient conditions; notifies RN/CM of changes.
• Counsels patients and families on disease processes and care plans.
• Maintains accurate clinical documentation per Medicare/federal guidelines.
• Travels to assigned locations and participates in on-call requirements.
