Job summary

CARetenders, part of the UnitedHealth Group family, is seeking a Licensed Practical Nurse to provide high-quality in-home patient care. The role focuses on performing skilled nursing tasks in patients’ homes and coordinating care with the health team.

You will document visits in electronic records, participate in case conferences, and adapt to various home environments while upholding infection control and regulatory standards.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification.
• Driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to work flexible hours as required to meet identified client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws and plan of care.
• Documents patient visits thoroughly in the electronic medical record and regulatory requirements.
• Participates in the agency's care coordination process and communicates with patients, families, physicians, and care coordinators.
