Job summary

Iowa Home Care in West Des Moines, IA is seeking a licensed Practical Nurse to provide in-home nursing care under a physician's plan. You will perform visits, assess patient conditions, record vitals, and administer medications in accordance with standards of practice.

Responsibilities include coordinating with the RN Case Manager, educating families on home safety, and documenting all care delivered. A valid Iowa LPN license and reliable transportation are required.

Qualifications

• 1–2 years of acute hospital or home health care experience preferred.
• Knowledge of nursing standards as set by the Iowa Board of Nursing.
• Strong communication skills and commitment to confidentiality.
• Valid driver's license and reliable transportation.
