Job summary

Phoenix Home Health is seeking a dedicated LPN to provide high-quality home health nursing in Stockton, MO and surrounding areas. You will assess patients, follow physician orders, adjust care plans, and document on-site with Roger's AI-powered support.

Qualifications

• Licensed practical nurse or registered nurse license (Missouri or Compact).
• Strong organizational and self-management skills.
• Excellent customer service and communication abilities.
• Valid driver’s license.

Responsibilities

• Assists in review and revision of the plan of care.
• Provides nursing care per physician's plan of treatment and patient plan of care.
• Observes, records and reports reactions to treatment and any condition changes.
• Provides ongoing instruction to client or family on plan of treatment.
• Participates in education programs and staff meetings.
• Monday through Friday 8am to 5pm with every 4 to 6 weeks, weekend rotation.
