Job summary

CHI Health at Home is seeking a compassionate Licensed Practical Nurse/Licensed Vocational Nurse to deliver direct patient care at home under approved treatment plans. You will collaborate with the RN Case Manager and health care team to ensure high-quality patient outcomes while adhering to clinical protocols.

Requirements include an accredited LPN/LVN program, state licensure, CPR/BLS certification, and 1 year of nursing experience (home care preferred).

Qualifications

• Graduate of an accredited practical nursing program and licensed to practice in the respective state.
• Current CPR or BLS certification with hands-on demonstration is required and must be maintained throughout employment.
• Minimum of one year of nursing experience; one year of home care or hospital nursing experience preferred.
• Excellent clinical skills and strong written and verbal communication abilities.
• Ability to work effectively within a multi-disciplinary team and build interpersonal relationships.
• Strong organizational and time management skills.
• Valid Driver’s license and compliant auto liability insurance coverage as per organizational policy.
• Experience with electronic health records (EHR) systems, such as HomeCare HomeBase, is a plus.

Responsibilities

• Collaborate with the RN Case Manager and healthcare team to ensure care per treatment plan and guidelines.
• Conduct patient visits per physician’s treatment plan, complying with regulations.
• Make skilled observations of patients' physical and mental conditions and document care provided.
• Report changes in patient condition to supervisors and attending physician promptly.
• Provide patient education to patients and caregivers on care plans and health management.
• Demonstrate sensitivity to patients’ needs, customs, and feelings.
• Participate in case conferences and in-services or special orientations.
• Complete and submit all required documentation accurately and promptly.
