Job summary

VitalCaring is seeking a licensed Practical Nurse (LPN) to provide direct in-home patient care under RN supervision, delivering individualized care plans and supporting positive outcomes. This role offers autonomy, flexible scheduling, and 1:1 patient interaction in patients' homes, with full benefits and opportunities for career growth.

Requirements include active LPN/LVN license in Florida, CPR certification, and at least one year of nursing experience; valid driver's license and reliable

Qualifications

Active LPN/LVN license in the state of practice.
Current CPR certification.
Minimum of one year nursing experience (home health, community, or med-surg a plus).

Responsibilities

Provide direct patient care in the home under the supervision of a Registered Nurse.
Execute individualized care plans and support positive patient outcomes.
Educate patients and caregivers on disease management and self-care.
Document care accurately and timely in the EMR.
Collaborate with physicians and interdisciplinary teams to ensure coordinated care.
Build trusted relationships with patients, families, and care teams.
