Job summary

Enhabit Home Health & Hospice in Oregon is seeking a compassionate Licensed Practical Nurse to visit patients in their homes and provide skilled nursing care under the direction of RNs and physicians.

You will collaborate with a care team, document care in patient records, and ensure safety and quality of care across diverse home settings. A valid LPN license in Oregon and reliable transportation are required.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seat belts at all times while driving.
• Dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed as a Licensed Practical Nurse in the State of employment.

Responsibilities

• Provide nursing care to patients in their homes under the direction of RNs and in accordance with physicians' orders.
