Care Advantage, Inc. is hiring multiple Licensed Practical Nurses (LPN) across Virginia to support our High Acuity Division. Our teams provide one-on-one in-home care, aiming to reduce depression and cognitive impairment with care plans developed by Registered Nurses.

Qualifications

• Eligible to work in the United States.
• CPR Certification and PPD Certification.
• 1+ year experience as an LPN.
• Valid LPN License with at least one year of work experience.
• Valid Driver's license.
• Reliable transportation.

Responsibilities

• Provide and implement direct patient care.
• Provide accurate and timely documentation consistent with the plan of care.
• Assess and provide family/caregiver education and information.
• Assist the RN or physician in performing specialized procedures and duties, including preparing equipment and materials for treatments, observing aseptic technique as required, and assisting the patient in learning appropriate self-care techniques.
• Participate in coordination of home health services, reporting identified needs for other disciplines to the RN and/or Clinical Supervisor.
