Job summary

The Riverside Medical Group - Peninsula - Primary Care Physicians in Hampton, VA is seeking a Licensed Practical Nurse to support preventive care and annual wellness visits within a multi-disciplinary team.

You will perform pre-visits, educate patients, assist with care gap closures, and ensure accurate billing details while collaborating with the remote health care coach team and case managers to improve patient outcomes.

• Education Program Graduate, Licensed Practical Nurse (Required).
• Experience 1 year Nursing experience (Required).
• LPN Certification (Virginia) Upon Hire; CPR/BLS Certification Upon Hire.

Responsibilities

• Uses practice guidelines to identify Medicare eligible patients for annual wellness visits.
• Ensure timely screenings and address care gaps to enable early interventions.
• Accurate billing details for services performed to the biller/coder.
• Provide patient education to promote health and wellness.
• Educate practice team with timely, relevant information for care goals.
• Perform pre-visits and patient outreach to close care gaps.
• Maintain accurate care documentation and patient safety.
• Address care concerns from wellness visits with providers or office manager.
• Complete charts same day of service and support multidisciplinary teamwork.

Job description

Hampton, Virginia Hiring Range $27.30 - $37.58/Hourly Actual pay is determined based on job-related factors such as relevant experience, education, credentials, skills, internal equity, and business needs.

Overview

Works independently within the practice and serves as a practice liaison in interactions with other RMG departments to include the remote health care coach team, the Complex Case Managers, and other health care and community services. Focuses on preventive health care as it relates to annual wellness visits and closing care gaps.

What you will do

• Utilizes Practice Dashboard to determine all Medicare eligible patients for Annual Wellness Visits and subsequent AWVs.
• Assures timely screenings of patients to allow early interventions on positive findings.
• Provides accurate billing detail on all services performed at the practice to the biller/coder.
• Provides effective education to patients that promotes health and wellness.
• Provides education and information to the practice team that is timely and relevant to the goals of the medical home.
• Performs pre-visits and patient outreach to assure closure in care gaps.
• Works quality dashboards to assure care gaps are being addressed.
• Care documentation is accurate, assuring patient safety.
• Care concerns from annual wellness visits are addressed timely to provider and/ or office manager.
• Completes charts with accuracy same day of service.
• Demonstrates the ability to build trusting collaborative relationships with a multi-disciplinary team to support patient care needs.
• Actively participates in multi-disciplinary team meetings.
