Responsibilities

• Uses Practice Dashboard to identify Medicare eligible patients for Annual Wellness Visits and AWVs; ensures timely screenings and accurate billing details.
• Educates patients and practice team on health and wellness aligned with the medical home goals.
• Performs pre-visits and outreach to close care gaps; uses dashboards to track care gaps; ensures accurate charting on the day of service.
• Builds collaborative relationships with a multi-disciplinary team and participates in team meetings.
