Qualifications

• Active, unrestricted LPN license in the state of practice.
• Graduate of an accredited Practical Nursing program.
• Experience in skilled nursing, long-term care, rehabilitation, or post-acute care preferred.
• Strong clinical assessment, medication administration, and documentation skills.
• Strong communication and teamwork skills.

Responsibilities

• Provide direct nursing care, medications, and treatments as prescribed.
• Monitor resident condition and report changes to the care team.
• Accurately document resident care and interventions.
• Collaborate with nurses, physicians, therapists, and other team members.
• Follow safety, infection prevention, and regulatory standards.
