Qualifications

• Active Illinois Licensed Practical Nurse license required.
• Long Term Care (LTC) experience preferred.
• Strong teamwork and collaboration within Nursing team.
• Effective communication with residents, families, and staff.

Responsibilities

• Deliver direct resident care.
• Administer medications.
• Assist with resident treatments.
• Collaborate with Nursing team.
• Document resident records and progress.
