Job summary

New Vista Nursing and Rehabilitation Center in Newark, NJ is seeking a licensed practical nurse (LPN) to join our care team on a part-time and full-time basis. You will administer medications, monitor patients, and help develop care plans with the healthcare team.

Requirements include an NJ LPN license, an Associate's degree, and at least 1 year of LTC experience. Strong communication and ability to assist with patient transfers are essential.

Qualifications

• Current LPN licensure in New Jersey.
• Associate's degree in Nursing or related field.
• Minimum 1 year of experience as an LPN in long-term care.

Responsibilities

• Administer medications and treatments as ordered by physicians
• Monitor and record patients' vital signs and medical conditions
• Develop and implement individualized care plans with input from the healthcare team
• Perform routine nursing tasks, such as wound care and vital sign checks
• Communicate effectively with patients, families, and healthcare team members
• Participate in quality improvement initiatives to enhance patient care and outcomes
