Job summary

Great Lakes Management is seeking a dedicated LPN to join our senior living teams in Minnetonka, MN. You will use your MN LPN license to assess residents, monitor care plans, and coordinate with physicians and families to ensure quality of life.

This role features evening hours (3PM-11:30PM), a competitive pay range of $31.00-$35.00 per hour, and a supportive environment with opportunities for professional growth in our team-oriented setting.

Qualifications

• Current LPN license in Minnesota is required.
• Minimum of one year experience in home health, assisted living or long-term care preferred.
• Good organizational skills with ability to work autonomously.
• Caring attitude toward elderly residents.

Responsibilities

• Oversee admission criteria, monitor residents, and advise on placement when needed.
• Maintain up-to-date care plans in accordance with state regulations.
• Track charts and clinical progress notes; communicate changes to physicians and families.
• Coordinate resident services with other providers and oversee medications and renewals.
• Ensure proper medication administration and document any needed corrections with staff.
• Collaborate with RN for alternating supervisory visits and team care.

LPN Responsibilities:

• Assures residents maintain admission criteria, monitor, and recommend alternative placement if necessary.
• Assures residents have up to date care plans per state regulations
• Monitors and maintains resident charts and clinical progress notes.
• Communicates to physicians, staff and family changes in resident's needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident's log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN.
• Works as a team player with all members of the residents' care team.
• Supervises medication practices and medical equipment being used in the building.
• Assures that staff clearly understand and follow all resident care procedures.
• Shares resident and family concerns with pertinent staff if appropriate.
• Works with manager to assure emergency procedures are in place

LPN Qualifications:

• Maintains a current state license as an LPN (Licensed Practical Nurse) per state regulations
• A minimum of one year's experience in home health, assisted living or long-term care is preferred
• Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision
• Exhibits a caring heart with a passion to care for the elderly
