Job summary

East Haven Nursing and Rehabilitation Center in the Bronx, NY, seeks a compassionate LPN for direct patient care, med administration, wound care, and vital signs monitoring under RN or physician supervision.

• Document patient care and progress accurately and thoroughly, in accordance with facility policies and regulatory requirements.
• Participate in patient education and discharge planning to ensure patients receive the necessary skills and support for successful transitions to home or other care settings.
