Job summary

Crisp Regional Health Services is seeking a Licensed Practical Nurse (LPN) to join the Physician Services team. The LPN collects data, provides comfort and care, and develops a care plan tailored to the patient’s developmental age and cultural needs.

The nurse interacts with patients, families, and office staff while maintaining professional nursing standards. Responsibilities include patient assessments, medication discussions, scheduling, and coordinating referrals.

Qualifications

  • Education: High school diploma or GED and graduation from an LPN program.
  • Experience: Up to one year of work-related experience or equivalent.
  • Licensure: Current LPN license and Basic Life Saving (BLS) certification; valid driver's license and proof of insurance.

Responsibilities

  • Collaborates with an interdisciplinary team and communicates effectively with patients, families, and staff.
  • Assesses patient condition and documents findings.
  • Uses professional judgment to treat patients and coordinates with the physician as needed.
  • Reviews health risks and nutritional and lifestyle considerations.
  • Discusses medications and tests with patients and assists with scheduling.
  • Schedules consultations and referrals and processes prescriptions as directed.
  • Answers patient questions and relays information; notifies about test results and follow-up visits.
  • Educates patients on care plans and evaluates understanding.
  • Maintains exam rooms stocked and reorders supplies.
  • Acts as a clinical leader and mentors staff; stays current with nursing innovations.
  • Ensures safety, infection control, and participation in performance improvement.
  • Presents a professional image to staff, patients, and families.
  • Attends training and continuing education; performs related duties as assigned.

Position Summary

Under the leadership of the department Manager, Physician Services, the Licensed Practical Nurse (LPN) is an active member of the Physician Services team that collects data and provides comfort measures and care to patients. The LPN is responsible for providing care that is appropriate for the patient’s developmental age and is culturally sensitive. The LPN formulates an appropriate plan for care and analyzes the effectiveness of interventions while effectively interacting with patients, families, and other medical office staff members. The LPN maintains standards of professional nursing.

Basic Qualifications
  • Education: Requires a high school diploma or a GED and graduation from a Licensed Practical Nurse program from an accredited vocational or community college.
  • Experience: Requires up to one year of work‑related experience, or any equivalent combination of education, training, and experience.
  • Licensure, Registrations & Certifications: Requires and maintains a current license as a Licensed Practical Nurse and a current certification in Basic Life Saving (BLS). (If BLS verification is not current upon hire, it must be obtained within three (3) months of hire.) Requires a current and valid driver’s license and proof of insurance.
Essential Job Responsibilities
  • Functions as part of an interdisciplinary team with positive interpersonal communication skills when communicating with patients, families, and the medical office staff.
  • Assesses patient’s condition and documents findings.
  • Uses professional judgment in treating patients and modifies the treatment and/or contacts the physician as appropriate.
  • Reviews relevant health risks, nutritional issues and corresponding lifestyle changes that are warranted.
  • Discusses with the patient and family members medications and medical tests and assists with their scheduling.
  • Schedules consultations and referrals as needed and calls in prescriptions as directed.
  • Answers patients’ questions and ascertains patients’ needs, acts upon routine requests, and relays information.
  • Notifies patients of test results as directed and any follow-up visits requested by the physician.
  • Communicates and reinforces patient care plan and education plan with patient and family and assesses the patient or family’s ability to comprehend.
  • Ensures examination rooms are stocked with supplies and reorders supplies as necessary.
  • Serves as a clinical leader, implementing nursing concepts, and mentoring other staff as necessary.
  • Remains current on innovations, developments, new procedures, and techniques in nursing care to provide quality nursing care services.
  • Assumes responsibility for safety, infection control and performance improvement as part of CRHS’s efforts to provide quality care and services. Performs all associated duties, responsibilities, and tasks.
  • Presents a professional image to CRHS’s medical staff, co‑workers, physicians, patients, and family members.
  • Attends professional development programs and in‑service training and meets the minimum requirement for continuing professional education.
  • Performs other related job duties as assigned.

  • Performs nursing care in accordance with all established department and CRHS policies and procedures as measured by feedback and observation from nursing, medical and management staff.
  • Performs all assigned patient care duties in a timely and accurate manner as measured by logs, charts, and observation.
  • Complies with all safety and infection control policies and practices in the delivery of direct patient care as measured by feedback and observation from nursing, medical and management staff.
  • Demonstrates reliability as measured by compliance with the CRHS Time and Attendance policy.
  • Demonstrates professional conduct and adherence to all safety, confidentiality, and HIPAA standards.
  • Promotes teamwork with the entire staff, as well as other contacts within the department as measured by observations of manager and co‑workers.
  • Adheres to the CRHS Standards of Behavior as measured by observations of staff, as well as outside and other inside customers.
  • Verbalizes all core competencies of the unit and department.
  • Complies with all regulatory requirements including the National Patient Safety Goals as observed by peers, supervisor, and all members of the healthcare team.
  • Actively participates as a member of the healthcare team by attending all mandated in‑services and a minimum of 75% of the unit staff meetings and employee forums.
