Responsibilities

• Execute the medication management plan with periodic reviews and physician communication as needed.
• Continuously assess resident needs and update care plans and documentation.
• Ensure resident wellness and safety with proper documentation of measures.
• Work with the team to meet resident needs and respond to emergencies promptly.
• Report incidents and changes in resident condition to Healthcare Director with documentation.
• Assist with meals in the dining room and monitor nutrition and diet acceptance.
• Maintain accurate records and documentation across departments.
• Provide shift-change reports and participate in Daily Log updates.
• Support CNA/RMA staff and provide motivation and direction.
• Attend required trainings and CEUs; comply with state certification requirements.
• Adhere to attendance policies and safe body mechanics for transfers.
