Job summary

River Edge Behavioral Health in Macon, GA is seeking a Licensed Practical Nurse for a Specialized Transitional Home program. You will provide in-home nursing care, administer medications, monitor vital signs, and support individuals learning daily living skills in a small home setting.

Join a team of clinicians to coordinate care with BCBA and other professionals, maintain records, and follow safety protocols to ensure a compassionate and secure environment for residents.

Qualifications

• Current Georgia license to practice as a licensed practical nurse.
• At least 1 year working in behavioral health/IDD or related field.
• Must have good computer skills.
• Communication skills and the ability to foster good relationships are a plus.

Responsibilities

• Administer medications as prescribed to patients in the home.
• Regularly assess and record patients’ vital signs to monitor stability.
• Assist individuals with daily personal care in the home.
• Treat and dress wounds and injuries that may arise.
• Assist in managing crises, using clinical knowledge to support patients.
• Coordinate care with BCBA and other healthcare professionals.
• Maintain accurate patient records in line with legal standards.
• Follow safety protocols to ensure a safe home environment.
• Perform housekeeping duties as needed.
