Job summary

Taconic Health Care Float Pool in Hudson Valley is seeking a Licensed Practical Nurse to provide high-quality nursing care, administer medications and treatments, and help ensure the safety, comfort, and well-being of residents. This role supports multiple skilled nursing and rehab communities with flexible shifts.

The LPN will work within an interdisciplinary team, supervise nursing assistants, and contribute to resident-centered care. CPR is required; NYS LPN license is mandatory.

Qualifications

• High School Diploma or equivalent; Associate's Degree preferred.
• Valid NYS LPN license required.
• Current CPR certification required.
• Experience in a skilled nursing facility or geriatric population preferred.
• Strong clinical, communication, and organizational skills.
• Knowledge of medication administration and infection control.
• Ability to supervise nursing assistants and work in an interdisciplinary team.
• Compassionate, resident-centered approach to care.

Responsibilities

• Provide nursing care in accordance with resident assessments, care plans, and facility policies.
• Administer medications, treatments, enteral feedings, and IV therapies within the LPN scope of practice.
• Monitor resident conditions and promptly communicate changes to providers, supervisors, and responsible parties.
• Accurately document care, treatments, observations, and medication administration.
• Supervise and support Nursing Assistants to ensure quality resident care.
• Maintain up-to-date resident care information and communicate pertinent information during shift changes.
• Follow infection control, safety, resident rights, and confidentiality standards.
• Participate in interdisciplinary care planning and quality improvement initiatives.
• Assist with admissions, transfers, and other nursing responsibilities as assigned.

REQUIREMENTS:
