Job summary

Maxim Healthcare is seeking a Licensed Practical Nurse (LPN) to deliver contracted home health nursing across multiple patient homes. You will manage ventilator, trach, and G/J-tube care with a focus on patient acuity in our markets.

Responsibilities include applying the nursing process, initiating plans of care, administering meds, and documenting outcomes while coordinating with prescribers and supervisors as patient status changes.

Qualifications

• Current LPN license in the practicing state.
• Current Health Certificate.
• Current BLS certification.
• One year of LPN experience.

Responsibilities

• Uses the nursing process to plan, implement, and evaluate patient care.
• Initiates the plan of care and liaises with prescribers as needed.
• Administers medications and documents effectiveness.
• Assesses patient changes and reports to physician or supervisor.
