Avante Care Management seeks a Licensed Practical Nurse in New York to provide resident care in a long-term care setting. The LPN reports to the RN Shift Supervisor and collaborates within an interdisciplinary team to ensure high-quality nursing services.

The role includes medication administration, care planning support, and adherence to state regulations. Strong communication and leadership are essential for this position.

Qualifications

• Current license as a Licensed Practical Nurse in New York is required.
• Graduate of an accredited LPN program.
• Must complete facility orientation and mandatory trainings.
• Long-term care experience required; supervisory experience preferred.
• Health assessment including two-step Mantoux test.
• In good standing with NYS Credentialing.
• Strong documentation and leadership abilities.
• Must communicate in English, verbally and in writing.

Responsibilities

• Provide nursing care to adult and geriatric residents per policies.
• Administer medications and treatments within LPN scope as prescribed.
• Supervise Nursing Assistants and prioritize resident care.
• Ensure compliance with state codes and proper documentation.
• Maintain resident rights and confidentiality.
• Respond to emergencies and participate in safety drills.
• Infection control and universal precautions.

Education

Licensed Practical Nurse (LPN) in New York

Graduate of an accredited LPN program

Health assessment requirements (Mantoux test)

Good standing with NYS Credentialing
