Job summary

Cambridge Rehabilitation and Healthcare Center in New Jersey is seeking an experienced Licensed Practical Nurse (LPN) to deliver patient-centered care on a fast-paced unit.

Responsibilities include medication administration, adherence to individualized care plans, and accurate documentation while collaborating with the interdisciplinary team.

Qualifications

• Active, unencumbered LPN license in New Jersey.
• BLS certification (CPR) required.
• Ability to manage multiple patients in a fast-paced setting.
• Strong clinical judgment and communication skills.
• Minimum 1 year of sub-acute/long-term care experience preferred.
• Experience with Point Click Care (PCC) preferred.

Responsibilities

• Provide patient care per standards of nursing practice and plan of care.
• Follow care plans developed by Nurse Manager and team.
• Administer medications and treatments per plan and policies.
• Monitor residents' conditions and report changes promptly.
• Maintain accurate documentation per physician orders and care plans.
• Support a safe, respectful, therapeutic environment for residents and families.
