Responsibilities include administering medications, monitoring residents, coordinating with the care team, and supervising CNAs to ensure high-quality patient care.

Qualifications

• Active Illinois LPN license (required).
• Graduate of an accredited nursing program.
• Experience in long-term care, skilled nursing, or rehabilitation preferred.
• Ability to work every other weekend.
• Strong clinical judgment and communication skills.
• Dependable, organized, and patient-focused.

Responsibilities

• Provide direct nursing care in a skilled nursing / long-term care setting.
• Administer medications, treatments, and IV therapy as ordered.
• Assess, monitor, and document resident conditions and care plans.
• Coordinate care with physicians, nurse practitioners, and interdisciplinary teams.
• Supervise and support CNAs.
• Ensure compliance with Illinois nursing standards and facility policies.
• Respond to changes in condition and provide clinical leadership during shifts.
