Qualifications

Active, unrestricted LPN license in the state of practice.
Recent graduates are encouraged to apply.
Experience with medication administration and patient care procedures.
Strong documentation, time management, and communication skills.
Familiarity with EHR and digital charting systems.

Responsibilities

Deliver direct nursing care to residents in skilled nursing and long-term care settings.
Administer medications and IVs as prescribed.
Implement and evaluate individualized care plans.
Record vital signs and monitor patient conditions.
Supervise and guide other nursing personnel when required.
Educate patients, families, and caregivers as directed.
