Job summary

Aveanna Healthcare, LLC is hiring a Licensed Practical Nurse (LPN) to deliver skilled nursing care in patients' homes under RN supervision. The position emphasizes documentation, patient education, and adherence to care plans.

You will work to achieve positive clinical outcomes in a compassionate, home-based setting. The role requires an active LPN license and CPR certification, with a typical full-time schedule from 6:00am to 2:00pm, Monday through Friday.

Qualifications

• Must have and maintain an active, unencumbered LPN license.
• Current CPR certification (hands-on component).
• TB skin test (within last 12 months).
• Six months of hands-on nursing experience preferred but not required.
• Must have reliable transportation.

Responsibilities

• Provide skilled nursing care under supervision of a Registered Nurse.
• Document and coordinate care according to the care plan and physician orders.
• Observe patient condition and communicate changes to supervisor/physician.
• Implement and update the nursing care plan as needed.
• Perform nursing actions to meet expected outcomes and ensure safety.
