Qualifications

• Licensed Practical Nurse graduate from an accredited nursing program.
• Certifications and LPN license required by state regulations.

Responsibilities

• Participate with interdisciplinary team to review, plan, coordinate and evaluate resident care.
• Document resident condition and nursing needs accurately; report observations timely.
• Develop and monitor nursing assistant assignments based on census and care level.
• Execute treatments and document reactions to medications and treatments.
• Generate, prepare, administer, and chart all medications.
• Initiate physician orders and coordinate changes with families.
• Ensure care plans are up-to-date and reflective of resident individuality.
• Coordinate admissions, discharges, and transfers to ensure quality service.
