Qualifications

• LPN license required by the state.
• CPR Certification required.
• Experience in long-term or rehabilitative care preferred.

Responsibilities

• Admissions, discharge and transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Follow infection control, emergencies, and center policies.
• Collaborate with interdisciplinary team to provide best outcomes.
• Report all abnormal changes in residents’ condition per center policy.
• Supervise care under state Nurse Practice Act and center procedures.
