Job summary

Riverside Health in Newport News, VA is seeking an educated and experienced Licensed Practical Nurse (LPN) to join our clinical team. You will perform focused patient assessments, collect vital signs, and support care plans under supervision of a physician or registered nurse.

Responsibilities include clinical documentation, lab proficiency, medication administration, vaccine administration, and coordinating with providers to address care gaps.

Qualifications

• Education Program Graduate, Licensed Practical Nurse (Required)
• 1 year Nursing experience (Preferred)
• Licensed Practical Nurse (LPN) per Virginia Dept. of Health Professions upon hire (Required)
• CPR/BLS Certification per AHA/ARC/IS Health Institute (Required)

Responsibilities

• Accurately completes all required clinical documentation per Riverside Medical Group standards.
• Maintains documentation for quality assurance including point of care testing, medication logs and annual wellness visits.
• Works with providers to assure care gaps are routinely addressed with each patient.
• Documentation is accurate, legible and completed same day.
• Knowledgeable and skilled in lab functions and lab proficiency testing.
• Maintains clinical competency by adhering to standard expectations for Riverside Medical Group.
• Updates and maintains patient records by interviewing the patient and confirming purpose of visit.
• Prepares patient for examination by obtaining vital signs (including manual if needed).
• Prepare and sanitize exam rooms before and after patient appointments.
• Administer medications and vaccines under physician supervision.
• Assists with processing prescription refill requests for approval and forwarding to pharmacies.
• Maintains medical inventory and ensures exam rooms are stocked.
• Ensures compliance with HIPAA, OSHA and scope-of-practice standards.
• Completes required competencies and education to maintain safe patient care.
• Provides patient education about medications prescribed.
• Demonstrates positive patient care by explaining the importance of patient registration and medical history forms.
• Provides patient follow up with urgency regarding lab calls, prescription fills and scheduling.
• Answers telephone calls from patients, pharmacies, physicians, vendors by close of business.
• Communicates lab/pathology results and schedules additional tests per provider order.
• Ensures proper reimbursement from insurance by pre-authorizing drugs, procedures and tests.
• Demonstrates ability to build trusted relationships with a multi-disciplinary team.
• Participates in multi-disciplinary team meetings as assigned.
