• Contributes to development and maintenance of a current, goal‑oriented plan of care and consults with other health professionals to assure patient/caregiver needs are met.
• Observes, assesses, documents, and reports patient responses to treatments and adjusts care priorities accordingly; collects and reports relevant data.
• Participates in unit/department enrichment activities and CQI initiatives to improve outcomes.
• Delivers a broad range of nursing treatments and supports patient care, including assessments, med administration, vital monitoring, and teaching.
• Assists patients and families during admission, orientation, and discharge.

Summary

Serves as a Licensed Practical Nurse for a Skilled Nursing Facility.

Responsibilities

• Contributes to the development and maintenance of a current, individualized, goal‑oriented plan of care and consults with other health professionals to assure that the assessed needs of patient/caregiver are met.
• Observes, assesses, documents, and reports patient response to treatments/therapies and teaching, and adjusts priorities of care according to patient/caregiver needs; collects, documents, and reports data regarding patient’s biophysical, psychosocial, environmental, self‑care, educational, and discharge planning needs.
• Participates in unit, service, and/or department enrichment activities which contribute to positive outcomes such as assigned committees, CQI activities/groups, and the orientation and education of other personnel.
• Provides a broad range of treatments/therapies to patients including, but not limited to, respiratory treatments, oral/NG/NT suctioning, and tracheostomy care; administers medications; observes cardiac monitors and respirators; assists physician with special procedures; monitors intravenous infusions; performs point‑of‑care testing; performs routine laboratory work; changes/irrigates catheters; provides specialized sterile dressings; and conducts patient/caregiver teaching based upon the established teaching plan.
• Provides assistance to patients and families during admission to the clinical area, orientation to the hospital/clinic experience, and discharge.
• Provides support to patients and families, verbally de‑escalates frightened or potentially aggressive patients, and facilitates age‑appropriate coping strategies for patients.

Education Requirements

• Completion of an accredited practical nursing program.

Licensure/Certification Requirements

• NC LPN License, BCLS

Knowledge/Skills and Abilities Requirements

• Language Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals; ability to write routine reports and correspondence.
• Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
• Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
