Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset in Nesconset, NY is seeking a Licensed Practical Nurse (LPN) to provide care for residents in the Skilled Unit and Long Term Care Unit. Duties include medication administration, including IVs, recording vital signs, and delivering treatments with outcome monitoring.

You will educate residents and families, coordinate with physicians and other health professionals, and supervise CNAs.

Qualifications

• Current LPN license is required.
• Experience in SNF/long term care preferred.
• IV administration experience is a bonus.

Responsibilities

• Provide care for residents in the Skilled and Long Term Care Units.
• Administer medications, including IVs, and monitor outcomes.
• Take and record vital signs and provide treatments.
• Educate residents and families; coordinate with physicians and other health professionals.
• Supervise CNAs during care delivery.
