Job summary

Lakeview Estates, part of Phoenix Senior Living, is seeking a Licensed Practical Nurse to join our team with leadership responsibilities in resident care.

The LPN will monitor residents, administer medications, document clinical changes, and support the Wellness Director. Weekend/evening shifts are required, with a flexible schedule to meet peak service needs.

Qualifications

• Must be a Licensed Practical Nurse
• A minimum of one year of elder care experience preferred
• Leadership capabilities
• Knowledge of Assisted Living and Dementia care or Skilled Nursing preferred
• Ability to work weekends, evenings, and flexible hours

Responsibilities

• Identifies ongoing needs and services of residents through assessments and PSPs.
• Ensures proper follow through and documentation for residents with changes in clinical care needs.
• Assists Wellness Director with completing clinical sections of monthly wellness visits and PSP updates.
• Informs Wellness/Executive Director of changes in medication or service levels.
• Assists with obtaining weights and vital signs monthly and during wellness updates.
• Assists with direct resident care as needed

PRN-- SHIFTS 6A-2P & 2P-10P-- URGENTLY HIRING!!

PURPOSE

The LPN is responsible for monitoring the health and wellbeing of our residents and provides leadership support and assistance with resident care and services.

Responsibilities include but are not limited to medication care administration, communication with resident support services and families and maintaining clinical quality assurance while leading, demonstrating and supporting all the elements of wellness which are core components of Phoenix programs and services.

Principle Duties And Responsibilities

LPN - Licensed Practical Nurse

Resident Care

Identifies on-going needs and services of residents through the assessment / Personalized Service Plan (PSP) process

Ensures proper follow through and documentation for residents with a change in clinical care needs

Assist the Wellness Director with Completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident monthly

Informs the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out

Assist with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates

Assist with direct resident care as needed

Education/Experience/Licensure- Certification

LPN - Licensed Practical Nurse

Must be a Licensed Practical Nurse

A minimum of one year of experience working with elders or disabled individuals, preferred

Must have demonstrated Leadership capabilities

Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred

Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times
